Position Summary

Penticton B.C. has an exciting opportunity for two part time Social Workers to join their team at the new Urgent Primary & Community Care Centre. The new urgent and primary care centre team in Penticon will include general practitioners, registered nurses, social workers, physiotherapists and medical office assistants. The Urgent Primary Care Social Worker provides comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, the Social Worker provides direct psychosocial care, counseling and group work, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self management. The Social Worker is responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.

Shifts will be 3 days one week and 2 days the next week. Shifts worked between Monday to Friday are 16:00 to 20:30, and shifts that fall on every second weekend (with Stats) are 11:30 to 18:30. This is a part time opportunity of 37.5 hours every pay period, with 2 vacant positions available.

Example: Week 1 Mon, Thur, Fri; Week 2 Tues, Wed 4.5 hours. Alternating weekends and some Stats 7.0 hours

Qualifications

Education, Training and Experience:
Bachelor’s Degree in Social Work from an accredited university.

Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.

Current full registration with the British Columbia College of Social Workers.

Current valid BC driver’s license.
